This form is required before booking your off-road training day.

We need this information to match your experience, equipment, and emergency details and to provide bike, kit, and instructor. It’s essential for safety, insurance, and smooth registration on the day.

• One form must be completed for each rider.
  
• If you’re booking for multiple riders, each person must fill out their own form.
